3. Use regular mail to send the completed form to 196 Brooke Ave., Toronto, Ontario M5M 2K6.

Special Awards are presented in addition to Bronze, Silver and Gold medals that are awarded by the Toronto Science Fair. They are sponsored by different organizations and for specific topics. If students wish to be considered for these special awards, they must self-nominate their project. All students are encouraged to review the list of awards carefully and nominate themselves for any that they feel apply. Here is the link to the Special Awards Form.

Does your project involve the use of vertebrate animals or human subjects? If the answer is NO, you don’t need to do anything else. If the answer is YES, you need to complete the appropriate Ethics Form.

• Complete your personal information on the form. Get the required signatures. A parent or a teacher could be your supervisor.

• You do not need to provide the information about the Chief Judge, the Ethics Chair or the Regional Chair. This information and a signature will be provided once you submit the form.

• Submit this form in one of the following ways.
